Personality: First and foremost, Manjiro Sano is a loving character. This love propels him into caring about the people around him, taking in their burdens, and he's incredibly sentimental to the point of keeping mementos of times and people in his life close to him at all times. As a kid, not only did he care about the Tokyo Maji, his gang, as an institution, but also their individual problems and hardships as well, going as far as to go to war with a rival gang for a single member's issue. As an adult, he's chosen to keep the people close to his heart far away, protecting them from a distance in which they'd not only be safe from others but from him as well. The toll it takes on him is visible, with his appearance shifting from a carefree kid to a withering adult, isolated in his own violence and doom, out of love so he doesn't hurt the ones who love him the most.
Interestingly, as Mikey cares deeply about the ones he loves, he couldn't really give many thoughts or cares about anyone who doesn't fit that bill. This is seen with him not batting an eye at and not caring about the violence committed to a friend's friend's girlfriend in a comma when a rival gang beat her up and committed other atrocities against her. Instead, his attention went to his friend's emotions towards the situation. As an adult, he has no issues whatsoever with the number of crimes Bonten commits, and neither does he have any emotions toward the traitors he ordered to be killed in front of him. This is the reasoning behind his crime - he hasn't murdered by his canonpoint, but he will not soon after do so.
The reason people stick to him in these things is his undying charisma. Something about being firm, but caring, and putting himself on the line for his people does that... And the fact he has never lost a single fight. People feel protected by his confidence and his skills, and the respect those gather makes him looked up to. A lot of this comes from Shinichiro, his older brother, who could easily win people over even without being able to throw a punch. He inspired Mikey in taking in his people and guide them with certainty. He's also charming and quirky, which absolutely helps - even when his dark impulses take over, there are people who would follow him without question or second thoughts, wanting a spot next to him on the top, undisputed.
With his dark impulse, Mikey has issues realizing what he does, until it's done. He can shoot his friends, in other timelines killing them in gruesome ways (strangling, immolation, so on), and he's awfully tired as Bonten, as he tries to balance his self-control and the desire to just destroy everything and everyone. As he shoots Takemichi, he reminds him that Mikey has shot him, tried to kill him, and thus deserves no compassion. In the end of the manga, he does kill Takemichi, only after realizing what he did. There's some clarity to what he does as wrong when he's not actively going through it, but he often battles it, believing it makes him stronger, while being concerned and terrified about other's safety around him.
Even as he deals with such a thing, he refuses to look to others for help. One of his biggest downfalls is literally taking all his traumas, all the burdens his lifestyle and members give him, and stuffing them in the very back of his mind. He won't talk about things in ways that allow him to process it, and as he is the boss and influence of many, he doesn't allow himself to look weak or show vulnerability whatsoever - as it would mean his group would be weak as a whole, and due to his mother's love being tied to how he fought, won, and reminded her of his father in that regard, he believes that love people feel about him is rooted in that, and he won't allow himself to lose that. Thankfully, he is extremely self-aware. He knows exactly where his heart lies, what sort of person he is, even if he doesn't fully understand it. He could explain with certainty what his dark impulse was, what his limitations are, and who were the people who acted as grounding to his impulsivity and trauma. Therefore, he can comprehend parts of himself with conviction.
The last thing worth noting is his intuition. He isn't a genius, but he has a unique and creepy sense of intuition that kind of manifests in a bit of sub-human ways. He can see things in a metaphorical way, noticing that Takemichi wasn't normal upon his first meeting because, through Mikey's eyes, Takemichi's back was bigger than anyone's, and how it was as if Takemichi was carrying many burdens.
He also can easily feel something is wrong with his good friends, as he deduced Mitsuya was in trouble, going as far as guessing his location correctly, because, you see, Baji and Shinichiro told him. Yes, the two dead people.
That sure is a thing.

Since he was a very young child, Mikey's been a martial arts prodigy. He lives and breathes fighting, growing up in a dojo did great to hone that experience in him. His fighting sense, analysis, flexibility, and reflexes are unmatched, and so is his signature kick to the temple of his enemies with unbelievable speed, which causes the majority of his enemies to get knocked out instantly. Great for him, because he's minuscule, and he uses his flexibility to close that gap between himself and taller enemies.
His favorite moves are kicks, and while his punches are terrific, his kicks can crush cars and also immobilize elite gang members with one of his signature temple kicks solely. He is regarded as the strongest member of the Tokyo Manji - "Invincible Mikey" is a respected title by all gangs, rivals, and the Manji alike, and remains true as of now in canon.
It's a challenging task to knock him out, but even when that happens, he comes back quickly and continues fighting. Even when injured, even when he's been beaten by a pipe repeatedly, he can continue fighting with his full strength.
If there is something Mikey absorbed from Shinichiro is his charisma. Unparalleled, blinding, powerful. It serves him well for guiding and leading the Manji and keeping the rival gangs respecting the group with no question about it. His charisma, however, has downsides. With the darkness within his heart growing and concealing right from wrong as he dissociates, it's easy to manipulate Mikey into growing into it. The charisma, then, is used as a weapon to create the Worst Gangs in history in most timelines, as expected and calculated by others.
Mikey himself says he harbors a 'darkness' deep within him. He states that sometimes, things don't make sense to him, and he can't tell good from bad or right from wrong, up from down, and his mind blanks. In some instances, he commits Atrocities and mentions that when he comes back to himself, he had already done them, and these acts are extremely violent in question. Therefore, he dissociates and there he goes in extreme destruction.
